Health and Safety Reminders
Important Reminder about Masks
Per the CDC and New York State Department of Health (DoH) guidelines, students and staff are not allowed to wear facemasks with vents or valves or “Gaiter Masks,” which are worn around the neck and pulled up over the mouth. When sending your child to school, please make sure that the mask sufficiently covers both the mouth and nose. Two-ply masks provide an increased level of protection and are highly recommended by the CDC and DoH.
The masks that are not allowed to be worn by students and staff are pictured below:

Baldwin Union Free School District now uses FinalForms, an online forms and data management service.
FinalForms allows parents to complete athletic participation forms for student-athletes, as well as the required New York State physical form, online. The data will be saved from season-to-season and year-to-year, and therefore, parents will never need to enter the same information twice.
Parents will be required to sign forms once per year and after any update.
Access FinalForms at https://baldwin-ny.finalforms.com.
