Device Loan Agreement
Families of students in grades 3 and 8 must complete the device loan agreement upon receiving their new devices. This applies to iPads, laptops, or any other technology device or equipment issued by the district.
The device loan agreement is emailed directly to the family email address on file. If you do not receive a link to complete the form via email, please contact your child’s building principal for further assistance.
Insurance Option for District Devices (Including Chromebooks)
Baldwin UFSD offers an optional insurance agreement with Smart Tech Insurance to provide peace of mind for any damage to your child’s Chromebook, as well as loss or theft of the device.
The price for the insurance policy is $32 and will be effective until July 1st, with an option to renew each school year.
Parents will need to purchase the insurance policy directly with Smart Tech Insurance using the instructions below. Also, all insurance claims are handled directly through Smart Tech. Baldwin UFSD DOES NOT need to be involved with any insurance claims.

To sign up for the 2026-2027 school year, simply go to Baldwin Schools - Smart Tech Insurance to enroll. The optional Chromebook insurance policy can be purchased starting July 1, 2026 until December 1, 2026.
