Health Exam (Physical) Form

• The required New York State Health Examination Form must be completed by in its entirety by a healthcare provider.

New York State Immunization Requirements 

All kindergarten, transfer students, and new entrants must provide evidence of immunization against diphtheria, Tetanus, Pertussis, hepatitis B, measles, mumps, rubella (German measles), polio, and varicella (chicken pox). 

All students enrolling in grades 6-12 for the 2024-25 school year must provide evidence of a tetanus, diphtheria, and pertussis (whooping cough) booster (Tdap). All students entering grades 7-12 for the 2024-2025 school year must provide evidence of receiving the Meningococcal vaccine.
